List of things to bring in...
1. Bring In your picture ID (current driver license, state ID or your passport)
2. Visa, Mastercard or cash payment (do not accept checks)
3. Immunization records if available (Doctor will advise you on getting the
needed immunizations during exam)
4. We will provide I 693 forms or you can download from our website.

I-693 Exam for any age is $285 Plus lab fees (Total Fee $395)

This Special Price of $395 includes a visit to finish necessary paperwork, exam, lab tests if required for syphilis, tuberculosis and gonorrhea. and one sealed envelope of properly completed I 693 forms for USCIS and a copy for your records.
It may cost you extra in time and money if some of your test results comes ABNORMAL and/or additional testing is advised by provider.
VACCINES (if needed) are NOT included in this price.

Extra Cost for Vaccines/Antibody Screen
Bring in your vaccine records if any. Doctor will review it and tell you what vaccines you may need. You can receive these vaccines thru your doctor's office or pharmacy as well
Prices for some of the vaccines for adults
$85 for Tdap/Td Vaccine needed if not having proof of this in last 10 yrs.
$130 for MMR vaccine or positive antibody screen
$65 for Flu vaccine...If exam is done during flu season (between SEP 1st thru MAR 31st) (high dose flu for age 65 + shot will cost more)
$ 300+ for Pneumonia vaccine (PCV 20) for age 65 + or advised by doctor. (PCV 15 vaccine (a bit cheaper) acceptable as well.
$195 for Varicella vaccine needed or positive Antibody screen
$96 for Hepatitis B vaccine (new requirement)
Vaccines for Kids (price varies)
Please bring in your vaccines records. Our doctors will review it and advise about getting vaccine(s) that are missing.
